A reminder -Our organization is interested in participating in the San Carlos Hometown Days Parade on Saturday, May 21, 2011. We understand that there is a $25 (non-profit organizations) or $50 (for-profit organizations) non-refundable entrance fee. Checks should be made payable to "San Carlos Hometown Days" and mailed to Hometown Days, 130 Dale Ave., San Carlos, CA 94070, together with a copy of this information form.

I agree to abide by the decision of the judges and all regulations of the Parade Committee, which reserves the right to finalize placement of entries and to refuse any entry. I further agree that I carry insurance assuring that this entry is completely safe from individual liability. My policy provides general liability for the one-morning event."
